Conviva Senior Primary Care Centers is seeking a skilled Family, Internal, or Geriatric Medicine Physician to join our team in Lake Worth, Florida. As a primary care physician, you will have the opportunity to take a hands-on approach to patient care, addressing their needs inside the exam room and looking beyond to understand the impact of social determinants on health outcomes.
Key Responsibilities- Provide high-quality, patient-centered care to 12-15 patients per day
- Participate in daily huddles with the Care Team to discuss patient care and medical expertise
- Act as a lead and active participant in the Care Team, providing medical expertise and guidance
- Manage a panel of patients with a focus on coordinated care and value-based relationships
- Doctor of Medicine or Osteopathic Medicine degree
- Board Certification or Board Eligibility in Family, Internal, or Geriatric Medicine
- Valid, unrestricted medical license in the state of desired employment
- Current, unrestricted DEA license
- Two to five years of directly applicable experience
- Experience managing Medicare Advantage panels and understanding of Best Practices in coordinated care environments
- Knowledge of Medicare guidelines and coverage
- Knowledge of HEDIS quality indicators
- Guaranteed base salary with 20% bi-annual quality incentive
- Total compensation up to $300,000
- Excellent benefit package, including health insurance, CME allowance/time, occurrence-based malpractice insurance, relocation and sign-on bonus options, 401(k) with employer match, life insurance/disability, paid time off/holidays, and minimal call
