**Job Summary:**

We are seeking a skilled Social Worker to provide social work services to our home health patients and contract clients in the Baltimore, MD area.

**Key Responsibilities:**

* Provide counseling and linkage with community services to patients as needed
* Perform assessments and assist in resolving social problems that may impede treatment, rehabilitation, and medical compliance
* Collaborate with other healthcare team members to understand psychosocial and emotional factors related to patient health problems
* Ensure compliance with home health regulations and contract requirements

**Requirements:**

* Bachelor's degree in Social Work or related field
* Experience in home health or social work
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
