Job Title: Community Health Advocate

We are seeking a compassionate and driven individual to join our team as a Community Health Advocate at El Centro Family Health. Our mission is to provide quality healthcare to the people of Northern New Mexico.

About the Role:
  • Advocate for patients and their families, connecting them with community resources and services.
  • Develop and implement health education programs, promoting disease prevention and health management.
  • Collaborate with the clinic team to ensure comprehensive care outcomes.
  • Manage appointment scheduling, call screening, and patient registration with a focus on customer service.
Requirements:
  • High school diploma or GED equivalent.
  • Minimum of two years' experience in a related field (preferred).
  • Strong interpersonal communication skills, with the ability to work effectively with diverse populations.
  • Organizational skills, attention to detail, and ability to multitask in a fast-paced environment.
  • Familiarity with Microsoft Office software, including Excel, Word, and Outlook.
Salary and Benefits:

$20.00 - $25.53 hourly, commensurate with education and experience, plus comprehensive benefits package.
