Key Responsibilities- Conduct Initial Visits: Conduct initial visits to assess the needs of patients and their families, and to identify potential care needs.
- Develop Care Plans: Develop and implement individualized care plans that address the unique needs and goals of each patient.
- Provide Skilled Nursing Care: Provide skilled nursing care and interventions to patients and their families, as needed.
- Collaborate with Interdisciplinary Team: Collaborate with the interdisciplinary team, including physicians, social workers, and other healthcare professionals, to ensure comprehensive care and support.
- Document Patient Care: Accurately and thoroughly document patient care and interactions, including progress notes, medication administration records, and other relevant information.
- Communicate with Patients and Families: Communicate effectively with patients and their families, providing education, support, and guidance as needed.
- Participate in Interdisciplinary Team Meetings: Participate in interdisciplinary team meetings to discuss patient care and progress, and to identify areas for improvement.
- Current RN License: Current RN license in the state of practice.
- Hospice Experience: Experience working in hospice care, preferably in a home health or hospice setting.
- Strong Communication Skills: Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to Work Independently: Ability to work independently, with minimal supervision, and to prioritize tasks and responsibilities effectively.
