Responsibilities:
  • Participate in Interdisciplinary Team (IDT) meetings to develop and implement comprehensive care plans that meet the unique needs of each participant.
  • Conduct thorough assessments of participants' psychosocial needs and provide supportive counseling to help them navigate life's challenges.
  • Facilitate hospital, rehabilitation, and nursing home admissions and discharges, ensuring a smooth transition for participants and their families.
  • Collaborate with community organizations and healthcare providers to ensure participants receive the necessary services and support.
  • Maintain accurate and timely documentation of participant interactions and care plans.
  • Develop and implement strategies to promote participant engagement and empowerment.
  • Work closely with the Director of Social Work and Behavioral Health providers to ensure guardianship is up to date and participants are educated on health care proxy and advance directives.
  • Perform other duties as required to support the care and well-being of our participants.
Qualifications:
  • Current Social Work licensure in the Commonwealth of Massachusetts at the Masters level (L.I.C.S.W. or L.C.S.W.)
  • Minimum of 1 year of experience in Social Work providing traditional Clinical or Case Management services with a geriatric population
  • Current C.P.R. Certification or ability to become certified
  • Strong verbal, written, and listening skills with the ability to multi-task in a fast-paced environment
