CenterWell Home Health is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Care Specialist. As a key member of our care team, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes.
Key Responsibilities- Provide comprehensive nursing care to patients with complex medical needs, including admission, case management, and follow-up visits.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
- Conduct thorough assessments, identify patient needs, and coordinate care with other healthcare professionals to ensure seamless transitions and optimal outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and timely interventions.
- Monitor patient progress, adjust care plans as needed, and document all interactions and interventions in accordance with regulatory requirements.
- Collaborate with interdisciplinary teams to ensure comprehensive care, including physical therapy, occupational therapy, speech-language pathology, and social work services.
- Participate in ongoing education and training to stay current with best practices, regulatory requirements, and industry developments.
- Diploma, Associate, or Bachelor's degree in Nursing
- Minimum one year of nursing experience, preferably in home health or a related setting
- Strong medical-surgical, ICU, ER, and acute care experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Valid driver's license, auto insurance, and reliable transportation
