Social Worker Job Description

Maintaining and/or improving the resident's ability to control everyday physical needs and mental/psychosocial needs is a key aspect of this role. The ideal candidate will identify individual social and emotional needs and develop and maintain individualized care plans.

Key responsibilities include participating in the planning of the resident's admission, community re-entry or transfer to another facility, and participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident.

The successful candidate will work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during their stay. Additionally, they will work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community.

Qualifications/Requirements
  • BSW or MSW preferred, or Bachelors degree in the related field
  • At least 3 years experience in the Social Work field required
  • Ability to work flexible schedule
  • PCC experience preferred
