Hospice Nurse Liaison

We are seeking a skilled and compassionate Hospice Nurse Liaison to join our team at a major Boston teaching hospital. As a Hospice Nurse Liaison, you will play a vital role in ensuring seamless care transitions for patients and their families.
Responsibilities:- Assess patients' needs and determine eligibility for hospice care
- Admit patients to hospice and conduct daily visits
- Develop and implement individualized care plans
- Collaborate with hospital nursing staff to coordinate care
- Serve as a liaison between staff, case management, patients, and families
- Participate in weekly interdisciplinary team meetings
- Educate patients and families about hospice care
- Minimum 2 years of experience as a Hospice RN in a Medicare-certified agency
- MA RN License
- RI RN License (or willingness to apply for it upon hire)
