About the Role

We are seeking a highly skilled Care Manager RN to join our team at CVS Health. As a Care Manager RN, you will play a critical role in delivering high-quality patient care and ensuring the best possible outcomes for our members.

Key Responsibilities
  • Assess and analyze patient medical and/or vocational status to develop a plan of care that facilitates optimal wellness and medical outcomes.
  • Implement and coordinate all case management activities relating to catastrophic cases and chronically ill members across the continuum of care.
  • Interact with patients, employers, providers, insurance companies, and healthcare personnel as appropriate to ensure seamless care coordination.
  • Communicate with patients and other stakeholders to provide ongoing case management services and ensure timely return to work or optimal functioning.
  • Prepare all required documentation of case work activities as appropriate.
Requirements
  • Active TX Compact license
  • Minimum 3+ years clinical practical experience, preferably in diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac care with Medicare members.
  • Minimum 2+ years CM, discharge planning, and/or home health care coordination experience.
  • Up to 55-75% travel to the Houston Methodist Hospital.
Preferred Qualifications
  • Ability to occasionally travel within a designated geographic area for in-person case management activities as directed by Leadership and/or as business needs arise.
  • Excellent analytical and problem-solving skills.
  • Effective communications, organizational, and interpersonal skills.
