Job Summary

We are seeking a highly skilled Registered Nurse (RN) to join our team as a Clinical Care Specialist. The successful candidate will be responsible for providing high-quality patient care in a home setting, working with pediatric and adult patients.

Responsibilities:

  • Develop and implement individualized care plans for patients, taking into account their unique needs and goals.
  • Conduct thorough assessments of patients' physical and emotional status, identifying areas for improvement and developing targeted interventions.
  • Collaborate with healthcare professionals and family members to ensure seamless coordination of care.
  • Administer medications and treatments as prescribed, monitoring patient responses and adjusting plans as necessary.
  • Provide education and support to patients and families, empowering them to manage their health effectively.

Requirements:

  • A current Registered Nurse (RN) license for the state in which you practice.
  • A valid Health Certificate, PPD or Chest X-Ray, and BLS card.
  • At least one year of prior experience as a Registered Nurse (RN).
