Job Overview:

We are seeking a skilled Registered Nurse to join our team as a Nurse Care Manager. In this role, you will be responsible for providing patient flow and outcome management services in collaboration with our inter-disciplinary teams.

Responsibilities:

* Collaborate with the healthcare team to achieve optimal efficiency in clinical, financial, and satisfaction outcomes
* Monitor and facilitate patient flow, and assist the care team in evaluating options, services, and interventions to meet patient's health care needs and length of stay targets
* Provide effective communication, collaboration, and coordination required for efficient clinical care progression

Requirements:

* Bachelor's of Science in Nursing required
* Current licensure to practice nursing in the State of Illinois
* Minimum two years of experience in an acute care setting required
* Previous leadership experience required

Preferred Qualifications:

* Advanced Cardiovascular Life Support (ACLS) - American Heart Association (AHA)
* Basic Life Support (BLS) - American Heart Association (AHA)
* NIHSS Stroke Scale Certification (NIHSS) - National Institute of Health Stroke Scale (NIHSS)
