Position Summary
The Chronic Care Navigator will play a crucial role in collaborating with healthcare providers to ensure comprehensive, patient-centered care. This position involves utilizing advanced management platforms to assist in the coordination of care for patients with chronic conditions.

Key Responsibilities

  • Gain a thorough understanding of primary care practice needs to facilitate effective population health management.
  • Act as a collaborative team member within the care team.
  • Work alongside healthcare teams to implement appropriate pre-visit and point-of-care processes.
  • Utilize the Chronic Care Management platform to support patients dealing with multiple chronic illnesses and aid in the continuity of their care.
  • Engage in quality improvement initiatives and redesign efforts aimed at enhancing population health outcomes.
  • Maintain and manage patient registries effectively.
  • Provide healthcare teams with essential data to address patient health requirements.
  • Assist practice staff in developing proactive interventions for targeted patient populations.
  • Ensure a positive experience for patients and their families through professional communication and efficient routing of inquiries.
  • Identify and report any discrepancies in data to the relevant personnel.
  • Foster teamwork within and across departments.
  • Participate actively in team meetings and discussions.
  • Adhere to all applicable policies and procedures while performing job functions.
  • Deliver necessary data to care teams to facilitate their processes.
  • Monitor patient attribution to ensure accuracy within the practice.
  • Perform additional duties as assigned.

Qualifications

  • A minimum of 3 years of experience in a relevant field, with a preference for background in population health.
  • Demonstrated problem-solving skills and the ability to manage multiple tasks effectively.
  • Exceptional communication abilities, both written and verbal.
  • Certification as a Medical Assistant from a recognized organization is preferred; however, significant experience in a primary care environment with a focus on quality and population health may be considered in lieu of certification.

Preferred Skills

  • Experience with EHR/EMR systems is highly desirable.
  • Bi-lingual candidates, particularly those fluent in English and Spanish, are preferred but not mandatory.
  • Preference will be given to candidates who are able to attend in-person training sessions.
