Job Summary

Codman Square Health Center is seeking a highly skilled Behavioral Health Clinician to join our team at the Randolph High School-based health center. As a core member of our multi-disciplinary care team, you will provide consultation, assessment, and bio-psychosocial evaluations to ensure the adequacy of behavioral health services for our patients.

Key Responsibilities
  • Provide consultation and assessment services to patients and families, including bio-psychosocial evaluations and crisis intervention.
  • Develop and implement client-centered treatment plans, including short-term treatment and linkage services.
  • Collaborate with the school-based care team, including the school nurse and school personnel, to identify behavioral health concerns and develop care-management processes.
  • Facilitate consultation between the multi-disciplinary care team, school personnel, and parents/guardians as needed.
  • Participate in clinical treatment team meetings to assist with treatment planning and provide comprehensive care for students and families.
  • Document services within the electronic health record, according to CSHC documentation guidelines.
Requirements
  • Master's degree in Social Work or related human services field.
  • Massachusetts license LCSW, LICSW, or LMHC.
  • Proficiency in speaking Haitian Creole or Spanish (preferred).
  • Culturally sensitive assessment and consultation skills.
  • Ability to work with diverse populations and provide age-appropriate interventions.
