Job Summary

The Advanced Practice Provider (APP) at American Family Care (AFC) plays a vital role in delivering high-quality patient care. As a key member of our healthcare team, you will work collaboratively with other providers and staff to ensure efficient patient flow and exceptional patient satisfaction.

Responsibilities
  • Conduct thorough health histories, review patient records, and develop comprehensive medical assessments to inform treatment plans.
  • Formulate medical and nursing diagnoses, and implement therapy or referrals to appropriate healthcare facilities and resources.
  • Provide emergency care and stabilization measures in critical situations, such as cardiac arrest, shock, or allergic reactions.
  • Analyze patient data to determine patient status, management, and treatment.
  • Offer guidance and education to patients, families, and significant others on health care and health promotion.
  • Perform other duties and responsibilities as assigned.
Qualifications
  • Excellent communication and interpersonal skills.
  • Professional appearance and attitude.
  • ANCC, AANP, or NCCPA Board Certification (with 1 year ER/UC experience preferred).
  • Active and current Physician Assistant or Nurse Practitioner licensure in the state of practice, DEA number, and state-controlled substance certificate, as required.
  • Able to multitask and work independently.
