Key Responsibilities
  • Lead a multidisciplinary team in developing and refining comprehensive care plans tailored to specific patient populations.
  • Implement clinical pathways, ensuring that the educational needs of patients and their families are met while monitoring for any deviations from established plans.
  • Collaborate with community partners to facilitate smooth transitions of care, ensuring effective communication with all stakeholders involved.
  • Work closely with Social Work, Utilization Review, and Clinical Documentation Specialists to optimize resources for patients and their families.
  • Conduct daily rounds to assess patient readiness for discharge, ensuring all learning objectives are achieved prior to discharge.
  • Maintain effective communication with healthcare providers, patients, families, and hospital staff to ensure a seamless care experience.
  • Act as a liaison between various departments to ensure timely receipt of pending orders and results.
  • Ensure follow-up appointments are scheduled prior to patient discharge and that discharge instructions are clearly communicated and understood.
  • Collaborate with physicians and the interdisciplinary team to guarantee safe and timely discharges.

Qualifications
  • Graduate of an accredited nursing program.
  • A minimum of two years of experience as a Registered Nurse is required.
  • Possession of a current Texas nursing license or a license from the Nursing Licensure Compact.
  • Current Basic Life Support (BLS) certification from an accredited organization.
  • Experience in care coordination or case management within an acute care setting is highly preferred.
  • Proficiency in reading, writing, and communicating in English is essential.
