Position Details

We are seeking a responsible, patient, and qualified Licensed Practical Nurse to join our team as a Care Coordinator. As a key member of our interdisciplinary team, you will play a vital role in reviewing patient charts to ensure accurate information and continuity of care.

Responsibilities
  • Conduct comprehensive assessments of patient medical, social, and emotional needs
  • Provide case management services to ensure optimal utilization of resources
  • Monitor and evaluate patient progress, adjusting care plans as needed
  • Collaborate with healthcare professionals to optimize patient outcomes
  • Oversee patient electronic medical records to ensure timely and effective care
  • Complete reports to identify areas for improvement and reduce hospitalizations
Qualifications
  • LPN license required
  • Minimum of 2 years of experience in acute care or managed care setting preferred
  • Proficient in conducting patient assessments and monitoring vital signs
  • Excellent communication and interpersonal skills to effectively collaborate with healthcare professionals
  • Experience with geriatric behavioral health preferred

Salary and Schedule

Salary range: $60,000 - $65,000 per year

Monday - Friday, 9:00am - 5:30pm
