  • Assist with navigating the healthcare system, including arranging appointments, referrals, and transportation as needed.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.

Requirements:

  • Current LPN (Licensed Practical Nurse) license in State of practice.
  • Minimum of 6 Months Long Term Care experience.
  • Geriatric behavioral health experience preferred.
  • Strong communication, interpersonal, and organizational skills.
  • Ability to work effectively in a team-oriented environment.
  • Proficiency in MS Suite, electronic medical records (EMR) and other relevant healthcare software.
  • Compassionate, patient-centered approach to care.
  • Basic Life Support (BLS) certification required.
