LPN Care Coordinator

We are seeking a skilled LPN to join our team at Hackensack Meridian Health. As a key member of our nursing staff, you will play a vital role in providing high-quality patient care and contributing to a positive and supportive work environment.
Key Responsibilities- Medical Management: Oversee medical management, including medication administration, ordering medication from contracted pharmacies, and documentation.
- Leadership Role: Take a leadership role in establishing a safe environment, providing personal supervision of residents, facilitating groups, and helping with day-to-day functioning of the milieu.
- Collaboration: Work closely with physicians to transcribe medication orders, order needed medication, and follow up to ensure proper medication and dosages are received.
- Care Coordination: Provide care to a resident population as delegated and supervised by the RN, documenting patient responses and interventions.
- Communication: Demonstrate effective therapeutic communication with patients, families, and significant others, responding promptly with tact and sensitivity.
- Admission and Discharge: Assist with the admission and discharge process under the supervision of an RN.
- Medication Management: Stock and order all medications for the Lodge from a contracted pharmacy, set up monthly medication books, and assist with maintaining insurance binders.
- Emergency Response: Intervene in emergency situations and initiate action necessary to maintain a safe, clean, and therapeutic environment.
- Documentation: Document nursing interventions and PRN medication in progress notes, including patient responses and lab results.
- Teamwork: Develop and maintain collegial relationships by sharing observations with other team members and working with the team to establish an environment that provides supervision and support.
- Education: Graduate of an accredited LPN program.
- Licensure: Licensed LPN.
