Restorative Nurse

We are seeking a skilled Restorative Nurse to join our care-giving team at Nivram Management. The ideal candidate will be responsible for developing and implementing restorative care programs, working closely with the interdisciplinary team to ensure high-quality patient care.
Key Responsibilities:
- Develop, organize, and implement restorative care programs to improve patient outcomes.
- Collaborate with the Director of Nursing and Assistant Director of Nursing to assess the quality of nursing care and identify areas for improvement.
- Conduct daily rounds to evaluate patient care, monitor nursing practices, and provide educational support to staff.
- Perform and oversee functional evaluations to assess patient progress and identify areas for improvement.
- Consult with nurses to develop and recommend restorative care plans.
- Ensure that all restorative care measures are documented and reflected in the care plan.
- Attend care planning conferences and provide recommendations to improve patient care.
- Report progress of restorative care measures and participate in Interdisciplinary/QAPI meetings to identify areas for improvement.
- Orient new staff on restorative care principles and practices.
- Coordinate with the Director of Nursing and Therapy to provide ongoing education and evaluation of direct care staff.
- Actively participate in physical restraint reduction and fall management programs.
- Set up documentation flow sheets for daily documentation of restorative care measures.
- Perform or supervise the performance of restorative care staff.
- Serve as a liaison with all therapists: physical, occupational, and speech.
- Participate in patient care planning and CMI meetings.
Requirements:
- 3+ years of Restorative Nurse experience preferred.
- Experience in SNF/LTC is preferred.
- Illinois RN or LPN license.
- Restorative Nurse Certification.
- CPR Certification.
- Ability to put patients first and ensure high-quality patient care in a compassionate and caring atmosphere.
