MDS Coordinator

The MDS Coordinator will play a crucial role in ensuring the accurate and timely assessment of residents in designated facilities. This position requires a strong understanding of federal and state guidelines regarding the RAI process and clinical reimbursement for services rendered.
Key Responsibilities- Evaluate facility compliance with federal regulations and targets for utilization.
- Identify areas of concern and assist facilities in developing effective action plans to address missing and late assessments.
- Review resident clinical records for supporting documentation on MDS coding, utilization, errors, assessments, and accurate transmission.
- Ensure timely completion of MDS assessments.
- Participate in weekly Medicare/PA meetings to discuss facility performance and address any concerns.
- Monitor and complete all Medicare admission/discharges assessments in a timely manner.
- Review hospital records, previous placement records, and interview family members or POA to gather pertinent information about residents upon admission.
- Provide education to staff on Medicare rules and regulations as needed.
- Review ABN tracking forms and NOMNOC to ensure timely and accurate documentation.
- Ensure physician certification process is followed and in compliance.
- Review all Medicare charting to ensure skilled need is documented for therapy and nursing daily.
- Review documentation to ensure Med B supporting documentation is present, including nursing documentation prior to therapy referral and MD orders.
- Ensure PDPM process is followed within CMS guidelines.
- Ensure all supporting documentation is present prior to coding MDS.
- Ensure timely submission/transmission of MDS and reconcile in PCC.
- Communicate and collaborate with Therapy Company, Administrators, Directors of Nursing, Restorative Nursing, and Compliance specialists to resolve any issues.
- Perform other duties as assigned by the Administrator, Director of Nursing, and Regional MDS Consultant.
