The Diabetes Nurse Practitioner will provide direct and indirect patient care in a hospital environment, working independently and interdependently with the medical, nursing, and ancillary staff.

The ideal candidate will have a Master's Degree in Acute Care Nurse Practitioner from an accredited school of nursing, at least two years' experience as a registered nurse or as a nurse practitioner, and Board-certification in Advanced Diabetes Management or the ability to take the exam when eligible.

Key Responsibilities:

  • Closely monitor the progress of patients with glycemic dysfunction (hyperglycemia or hypoglycemia).
  • Intervene when predetermined practice guidelines from American Diabetes Association (ADA), American Association of Clinical Endocrinologist (AACE), and American Healthways Corporation (AMHC) are not followed.
  • Collaborate with attending physicians to assure the achievement of performance measures for the diabetes population.
  • Interview patients and their family members and review records to obtain complete health history and document information appropriately in patient medical records.
  • Perform and document assessments/reassessments of patients using appropriate age-specific techniques thoroughly, accurately, and in a timely manner.
  • Evaluate patient progress/response toward expected outcomes, documents findings in progress notes, and shares findings with responsible physician by verbal or written report.
  • Provide age-appropriate health counseling and guidance to patients and their families in an informative and compassionate manner.
  • Initiate and interpret diagnostic tests and procedures relevant to the patient's current status.
  • Order and/or adjust frequency of blood glucose monitoring.
  • Order diabetes-related labs (A1C, creatinine, etc.).
  • Make adjustments in diabetes medications, including changing sliding scale, ordering an insulin drip, initiating insulin, restarting and making adjustments in home regimen, discontinuing diabetes medication when contraindicated, initiating or reinstating oral medications, ordering/providing diabetes self-management training, prescribing blood glucose monitors and supplies, and initiating endocrinology/diabetology consults for patients with unresolving hyper/hypoglycemia and/or diabetes complications affecting the course of treatment.

Requirements:

  • Master's Degree in Acute Care Nurse Practitioner from an accredited school of nursing.
  • At least two years' experience as a registered nurse or as a nurse practitioner.
  • Board-certification in Advanced Diabetes Management or the ability to take the exam when eligible.
  • Current certification of fitness from Board of Nursing as a nurse practitioner.
  • B.C.L.S. certification required.
