About the Role:

We are seeking a highly motivated and dedicated Registered Nurse to join our multidisciplinary team at LifeLong Medical Care. As a Primary Care Nurse Specialist, you will play a critical role in providing exceptional primary care services to our patients with complex medical conditions.

Key Responsibilities:

  • Coordinate team-based care by focusing on the intersection between medical and social needs of patients
  • Act as a resource for support staff for triaging walk-in and telephonic care
  • Facilitate chronic disease management activities
  • Coordinate primary care clinic services with other providers and agencies
  • Use clinician-approved standing orders to order labs and preventive screenings, and to refill routine medications

Requirements:

  • Bachelor's or associate degree in Nursing
  • Current and valid California Registered Nurse license and BLS certification
  • Proficient in Microsoft office suite
  • Knowledge of electronic health records (EHR), Epic preferred

Preferred Qualifications:

  • Minimum one-year experience in community health as an RN
  • Knowledge of outpatient care, triage, and clinic flow
  • Demonstrated ability and sensitivity to provide services to unhoused, undocumented, substance users, HIV (AIDS) infected, disabled, and psychologically impaired
