Salary: $62,500 - $67,500 per year

Job Description:

We seek a responsible, patient, and qualified Licensed Practical Nurse to assist with reviewing patient charts. You will review patient records to ensure accuracy and reflect conditions and treatment requirements.

Key Responsibilities:

  • Conduct comprehensive assessments of patient medical, social, and emotional needs
  • Provide case management services for continuity of care and resource utilization
  • Monitor and evaluate patient progress, adjusting care plans as needed
  • Collaborate with interdisciplinary teams to optimize patient outcomes
  • Oversee patient EMR for proper care
  • Complete reports to reduce patient gaps and hospitalizations

Requirements:

  • LPN license required
  • Minimum 2 years of experience in acute care or managed care setting preferred
  • Proficient in conducting patient assessments and monitoring vital signs
  • Excellent communication and interpersonal skills for healthcare professional collaboration
  • Experience with Geriatric behavioral health preferred
