Job Title: LPN Care Specialist

We are seeking a skilled LPN Care Specialist to join our team at MJHS Home Care. As a vital member of our healthcare team, you will provide advanced clinical services and emotional support to patients recovering from illness, injury, or surgery in the comfort of their own homes.

Responsibilities:
Implement care plans, evaluate treatment, and document visits electronically within 24-48 hours.
Work closely with the Coordinator of Care Nurse to ensure seamless patient care.
Utilize strong decision-making capabilities to oversee the recovery process for patients in their home environment.

Qualifications:
NY State LPN License required.
Minimum one year Med/surg experience in a hospital or Longterm Care facility necessary.
Valid Driver License and Car required.
Bilingual Spanish, Mandarin or Cantonese preferred.
