Job Title: Foster Care Social Worker

At Axesspointe Community Health Center, we are seeking a highly skilled and compassionate Foster Care Social Worker to join our team.

Job Summary:

The Foster Care Social Worker will be responsible for providing a range of services to birth families, foster families, and adoptive families, including:

  • Conducting comprehensive assessments of individuals and families to identify their needs and develop case plans
  • Providing support, training, and education to foster families on foster care policy and procedure
  • Recruiting and retaining foster families
  • Providing crisis intervention services, including being available on-call as needed
  • Reporting suspected child abuse or neglect as required by state child protection statutes
  • Participating in phone calls, home visits, court hearings, and other appointments related to FCS clients
  • Developing, implementing, and monitoring case plans and services being rendered to clients
  • Preparing and maintaining clear and concise reports and documents according to agency/state licensing policies and procedures
  • Attending all court hearings as required and providing accurate testimony and recommendations regarding case progress and services rendered to children and families

The ideal candidate will have a strong understanding of child welfare policies and procedures, excellent communication and interpersonal skills, and the ability to work independently and as part of a team.

Responsibilities:

The Foster Care Social Worker will be responsible for the following:

  • Providing services to birth families, foster families, and adoptive families
  • Conducting assessments and developing case plans
  • Providing support, training, and education to foster families
  • Recruiting and retaining foster families
  • Providing crisis intervention services
  • Reporting suspected child abuse or neglect

The Foster Care Social Worker will work closely with other staff members, including program managers, social workers, and support staff, to ensure that services are being rendered to children and families appropriately.

Requirements:

To be successful in this role, the Foster Care Social Worker will need to have:

  • A strong understanding of child welfare policies and procedures
  • Excellent communication and interpersonal skills
  • The ability to work independently and as part of a team
  • A strong commitment to providing high-quality services to children and families

The Foster Care Social Worker will be required to maintain confidentiality and handle sensitive information with discretion.

Working Conditions:

The Foster Care Social Worker will work in a fast-paced environment with multiple priorities and deadlines. The position will require frequent travel to meet with clients and participate in court hearings and other appointments.

Education and Experience:

The ideal candidate will have a Bachelor's degree in Social Work or a related field and at least 2 years of experience working in child welfare or a related field.
