Key Responsibilities:
- Conduct comprehensive in-home health assessments to identify and address the unique needs of our members.
- Develop and implement personalized care plans to promote optimal health and well-being.
- Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
- Provide education and support to members and their families to empower them to take an active role in their healthcare.
Requirements:
- NP license and national certification (ANCC or AANP) in good standing.
- Family, Geriatrics, Adult, Adult-Gerontology Primary or Adult-Gerontology Acute Care certification.
- Access to reliable transportation to travel to client and/or patient sites within a designated area.
Preferred Qualifications:
- 1+ years of clinical experience in a gerontology or home health care setting.
- Experience working with geriatric or Medicare populations.
