- Registered Nurse with a current license to practice in the state of Texas
- 5+ years of experience in a physician's office, clinical, hospice, or hospital setting
- Experience in advanced care planning and end-of-life discussions
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Hospice and Palliative Care Nursing certification
- 2+ years of hospice experience
- Bilingual (English/Spanish) language proficiency
