Hospice Liaison

About the Role:

Amedisys, a leading home health and hospice company, seeks a Hospice Liaison to join their team. This role involves developing relationships with healthcare professionals, patients, and families to educate them about home care services.

Key Responsibilities:

• Develop and maintain relationships with key referral sources, such as hospitals, nursing homes, and physician offices.
• Coordinate patient admissions and obtain required documentation.
• Conduct on-site visits to assess patient needs and identify barriers to care.
• Keep accurate records of patient interactions and referrals.
• Achieve admissions, product, and payor-mix goals.

Requirements:

• At least one year of experience in a healthcare-related business development/outreach role.
• A bachelor's degree is preferred, but not required.
• Reliable transportation, current driver's license, and liability insurance.

Travel Requirements:

This role will travel 50% or more within the assigned territory.

Compensation:

Amedisys offers attractive pay, including a salary range of $80,000 - $85,000 plus bonus potential.
