Job Title: Home Care Registered Nurse

We are seeking a compassionate and skilled Home Care Registered Nurse to join our team at Trinity Health of New England at Home. As a Home Care RN, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.

Job Summary:

The Home Care Registered Nurse is responsible for providing primary nursing care to patients in their homes, as ordered by the physician and in accordance with our organization's policies and procedures. You will work collaboratively with our Population Health Case Managers and other healthcare professionals to develop and implement individualized care plans that promote patient independence and well-being.

Responsibilities:
  • Conduct thorough assessments of patients' medical and functional needs, identifying areas for improvement and developing targeted interventions.
  • Collaborate with patients, families, and healthcare teams to develop and implement comprehensive care plans that address physical, emotional, and social needs.
  • Provide skilled nursing care, including wound care, medication management, and education on disease management and self-care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and coordination.
  • Document patient progress and outcomes in a timely and accurate manner, using standardized documentation tools and guidelines.
Requirements:
  • Graduate of an accredited nursing program, with a Bachelor's degree in Nursing (BSN) preferred.
  • Licensure as a Registered Nurse in the State of Connecticut.
  • At least one year of experience as a professional acute care nurse, with home care experience preferred but not required.
  • Current driver's license and reliable transportation.
