Job Title: Home Care Registered Nurse

We are seeking a skilled and compassionate Home Care Registered Nurse to join our team at Trinity Health. As a Home Care RN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver exceptional outcomes.

Job Summary:

The Home Care Registered Nurse is responsible for providing primary nursing care to patients in their homes, as ordered by the physician and in accordance with our organization's policies and procedures. This includes assessing patient condition and risk, developing and implementing individualized care plans, and providing education and support to patients and their families.

Key Responsibilities:
  • Assess patient condition and risk at admission, at required time points, and as needed, utilizing appropriate standardized documentation.
  • Develop and implement individualized care plans in collaboration with the physician, Population Health Case Manager, and patient.
  • Provide education and support to patients and their families to promote independence and improve health outcomes.
  • Collaborate with the interdisciplinary team to coordinate care and services.
  • Maintain accurate and timely documentation of patient care and services provided.
Requirements:
  • Graduate of an approved nursing education program, BSN preferred.
  • Licensure as a Registered Nurse in the State of Connecticut.
  • One (1) year experience as a professional acute care nurse.
  • Home care experience preferred but not required.
  • Must have current Driver's license and reliable transportation.
