Job Summary:

As a Primary Care Provider at Community Health Center, you will play a vital role in delivering high-quality medical services to underserved populations. You will work in an innovative practice environment that utilizes Advanced Access scheduling, providing a full scope of medical services. Our team offers a blend of individual and population-based care, and we are committed to improving health outcomes for special and vulnerable populations.

Key Responsibilities:
  • Provide comprehensive medical services, including preventive care, diagnosis, and treatment of various health conditions.
  • Collaborate with a multi-disciplinary team to develop and implement patient care plans.
  • Participate in practice-based research and quality improvement initiatives to advance healthcare delivery.

Requirements:
  • MD/DO degree from an accredited medical school and completion of a family practice residency program.
  • Board certification within 24 months of employment and licensure or eligibility for licensure in the State of Connecticut.
  • Bi-lingual skills in English and Spanish are preferred.
