We are seeking a skilled RN Triage Specialist to join our team at SSM Health. As a key member of our Internal Medicine Department, you will play a vital role in providing exceptional patient care and support.
Key Responsibilities- Implement and monitor patient care plans to ensure optimal outcomes.
- Coordinate with healthcare teams to provide seamless patient care.
- Communicate effectively with patients, families, and healthcare professionals.
- Assess and coordinate patient discharge planning needs.
- Apply evidence-based practice and scientific knowledge to inform nursing decisions.
- Collaborate with physicians and other healthcare professionals to ensure high-quality patient care.
- Perform other duties as assigned to support the team.
- Graduate of an accredited school of nursing.
- Basic Life Support (BLS) certification.
- Registered Nurse (RN) licensure in the state of practice.
This role is a full-time position, working Monday through Friday, 8am to 4:30pm. The RN Triage Specialist will work in a fast-paced, dynamic environment, collaborating with a multidisciplinary team to provide exceptional patient care.
