Job Summary:

The Spine and Pain Nurse Navigator is a critical role within our healthcare organization, responsible for guiding patients through the comprehensive spine and pain treatment plan designed for their unique needs. This position requires a strong understanding of patient care, education, and advocacy, as well as excellent communication and collaboration skills.

Key Responsibilities:

  • Triage new referrals to ensure patients are connected with the correct specialty provider and assist in obtaining their first appointment.
  • Provide education to patients and families in a clear and concise manner, answer questions, and empower them to take an active role in their spine and pain journey.
  • Act as a patient advocate, ensuring their needs are met and their voices are heard.
  • Facilitate collaboration with multidisciplinary health team members to ensure seamless care coordination.
  • Focus on delivering an exceptional patient experience, meeting the unique needs of each patient while striving for efficiencies in patient care and flow.
  • Ensure follow-up with patients to identify and address any additional resources or support needed.
  • Work closely with providers as the primary patient liaison to support the physician-patient relationship.
  • Assist with data collection, analysis, and identifying areas of improvement to enhance patient care and outcomes.

Requirements:

  • Experience: 2-3 years' experience in a healthcare setting required.
  • Preferred Experience: 3 years of experience in spine or pain management.
  • Required Education: Graduate from an approved school of nursing with a current RN license in the state of practice.
  • Preferred Education: Bachelor's Degree in Nursing.
  • Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiac Life Support) required within 90 days of hire/transfer.
  • Preferred Certification: Pain Management Nurse Certification.
  • Preferred Skills: Biopsychosocial assessment skills, time management skills, and the ability to effectively utilize resources to meet patient needs.
  • Preferred Other: Public speaking skills, ability to easily understand and integrate new information, and experience with evidence-based best practices and protocol development/improvement.
