Job Summary:

We are seeking a highly skilled and experienced LPN to join our team as a Resident Care Manager. In this role, you will be responsible for coordinating daily nursing activities and care, as well as training and coaching other staff members.

Key Responsibilities:

  • Implement the established plan of care and delegate tasks in accordance with scope of practice.
  • Monitor work assignments, provide coaching, training, and feedback to clinical team members.
  • Assist with new admissions, transfers, and discharges, and oversee ongoing resident care.
  • Contribute to resident care conferences and evaluate the interdisciplinary plan of care for each resident.
  • Observe, audit, and review each resident's condition, status, and care requirements.

Requirements:

  • Active LPN license in the state of Oregon.
  • Minimum of 1 year working in geriatric nursing, including experience with medication administration, delivery systems, and pharmacies.
  • One year in a supervisory/management position, with experience with budgets, staff development, training, and scheduling.
