Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team in a Long Term Care setting. As a LPN, you will be responsible for providing high-quality patient care and working collaboratively with our interdisciplinary team to ensure the best possible outcomes for our residents.

Responsibilities
  • Develop and implement individualized care plans to meet the unique needs of each resident
  • Provide direct patient care, including administering medications, performing wound care, and assisting with daily living activities
  • Collaborate with the interdisciplinary team to coordinate care and ensure seamless transitions
  • Participate in quality improvement initiatives to enhance patient care and satisfaction
Requirements

To be successful in this role, you will need:

  • A current LPN license in the state of Maryland
  • Basic Life Support (BLS) certification
  • Experience working in a Long Term Care setting, preferably with a focus on rehabilitation and restorative care
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
