As a Rehabilitation Therapy Assistant, you will play a crucial role in supporting the implementation of rehabilitation care plans. You will monitor and report on patient progress to the supervising Physical Therapist and collaborate with other healthcare professionals.
- Deliver direct patient care in alignment with the prescribed treatment plans from authorized healthcare providers.
- Ensure timely submission of necessary documentation in accordance with departmental protocols.
- Maintain accurate progress notes and comprehensive discharge summaries for assigned patients.
- Act promptly in accordance with established emergency procedures to ensure patient safety and care.
- Engage in quality improvement initiatives, including chart reviews and related activities.
- Participate in family meetings or conferences that focus on the care and management of assigned patients.
- Perform additional duties as required, recognizing that all responsibilities listed are essential functions of the role.
- Associate's Degree in a relevant field is required, or an equivalent combination of education and experience.
- A valid license to practice as a Physical Therapy Assistant in New York State is mandatory, along with any necessary specialized certifications.
3 to 3 years of relevant experience is required for this position.
