Effective communication with the overseeing physician and other healthcare professionals is essential to ensure a cohesive home health care strategy.
Key Responsibilities:
- Collaborate with patients, their representatives, caregivers, and physicians to develop, implement, and assess individualized care plans.
- Deliver services as prescribed by the physician, ensuring consistent communication with all involved healthcare providers.
- Administer direct occupational therapy interventions in alignment with the established care plan.
- Maintain comprehensive records of client visits, including evaluations, care plans, and responses to therapy, while facilitating discharge planning when necessary.
- Engage in case management discussions with nursing staff and other team members to integrate the occupational therapy program within the overall care plan.
- Provide education and support to patients and their families regarding the occupational therapy process.
- Assess patient and family needs for additional home health services and facilitate referrals as appropriate.
- Contribute to the agency's quality assurance and performance improvement initiatives, as well as participate in sponsored training sessions.
- Valid Driver's License and access to a reliable vehicle.
- Bachelor's Degree in a relevant field.
- Minimum of one year of experience in occupational therapy.
- Requires significant manual dexterity and motor coordination.
- Involves exposure to occupational hazards.
- May require lifting and exerting up to 50 lbs. of force.
- Potential exposure to various environmental factors.
