Job Summary

The Central Intake Nurse LPN plays a vital role in coordinating and facilitating the delivery of homecare services. This position requires strong communication and organizational skills to ensure seamless collaboration with the homecare team, hospital discharge planning staff, and other referral sources.

Key Responsibilities
  • Facilitate referrals and coordinate skilled and non-skilled services
  • Collect and input data to ensure accurate and timely delivery of homecare services
  • Demonstrate positive interrelationships and service excellence in the performance of duties
  • Meet or exceed expectations of internal and external service groups
Requirements
  • HS - High School Diploma or GED required
  • 1 year of experience in Medical/Surgical Nursing required
  • 1 year of experience in a Home Care setting preferred
  • Licensed Practical Nurse upon hire required
  • NYS Drivers License upon hire required
