Key Responsibilities:

  • Assess physician medication orders for appropriateness regarding drug selection, dosage, potential interactions, and administration routes. Recommend therapy modifications to physicians and facility staff as necessary.
  • Maintain a comprehensive patient database, including relevant medical history, clinical data, and current medications (including over-the-counter and home remedies).
  • Ensure that necessary laboratory tests for monitoring high-risk conditions are ordered, assessed, and communicated with the healthcare team.
  • Collaborate with the healthcare team to provide clinical and technical support as required.
  • Document all interactions with physicians, nurses, and other healthcare team members, as well as patients and their families, in the medical record.
  • Implement policies and procedures for the safe procurement, storage, distribution, and disposal of pharmaceuticals.
  • Obtain and maintain all required documentation in compliance with federal and state regulations.
  • Notify the Pharmacy Manager of any medication-related errors.
  • Assist facility staff with procedures related to the safe and effective management of pharmaceuticals.
  • Perform additional tasks as assigned.
Qualifications:

  • Bachelor's Degree in Pharmacy; Pharm D preferred.
  • Minimum of one year of experience in long-term care.
  • Licenses/Certifications: As required by government regulations and business needs.

  • Detail-oriented, goal-driven, team-oriented, and possess strong oral communication skills.
