Job Title

A Home Healthcare Specialist is needed to provide high-quality nursing care to patients in their homes.


About the Role

We are seeking a dedicated and passionate individual who will work closely with patients, families, and healthcare teams to provide comprehensive care services.

  1. Develop and implement personalized care plans to meet patient needs.
  2. Provide ongoing assessments and evaluations to ensure optimal patient outcomes.

Requirements
  • Graduate of an accredited school of nursing required.
  • Current license as a Licensed Practical Nurse in the State of Hawaii required.
  • Minimum one-year recent experience in healthcare.
  • Current BLS certification.

Estimated Salary: $90,000 - $120,000 per year.
