HOW YOU'LL MAKE A DIFFERENCE:
As a Physical Therapist (PT), you will embody care and compassion in your role. You will collaborate with an interdisciplinary team dedicated to delivering high-quality, compassionate care and achieving positive outcomes for our patients. Your responsibilities will include providing home-based therapy as prescribed by the attending physician, engaging with patients' families, and being an integral part of their recovery journey. Home care offers a unique perspective on how to effectively support patients in restoring their health.

HOW YOU'LL WORK:
You will deliver comprehensive skilled services as directed by the attending physician. Educating patients and their families while ensuring patient safety will be a key part of your role. You will be responsible for accurate documentation and compliance with all legal regulations.

MAJOR AREAS OF RESPONSIBILITY:
  • Plan of Care: Deliver skilled physical therapy as ordered by the attending physician. Observe, document, and communicate patient reactions, changes, and progress to the attending physician and relevant agency staff. Conduct OASIS assessments to create a personalized plan of care and adjust as necessary.
  • Patient Outcomes: Treat patients to alleviate pain, restore function, and maintain optimal performance through various physical means, including exercise, massage, heat, and water.
  • Minimize Patient Risk: Reduce re-hospitalizations by prioritizing visits for high-risk patients and educating them on preventive measures and decision-making.
  • Family Educator/Advocate: Guide patients and families on managing diseases or injuries throughout the physical therapy process. Instruct on the proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other assistive devices.
  • Supervision: Oversee therapy services provided by PTAs and, when appropriate, nurse assistants.
  • Communication/Collaboration: Maintain communication with PTAs and nurse aides regarding supervisory visit outcomes.
  • Policies: Complete all clinical documentation in accordance with agency protocols and Medicare/Federal guidelines.
  • Rules and Regulations: Understand and adhere to agency policies, procedures, rules, and regulations, and communicate any changes in schedule or availability to schedulers or supervisors.
  • Operations: Participate in in-service training and mandatory agency meetings.

  • Attention to detail is crucial, along with being observant and following instructions
REQUIREMENTS:
  • Current license as a Physical Therapist in the state of employment.
  • Valid driver's license and auto insurance in your name.
  • Ability to meet all physical demands of the role.
