HOW YOU'LL MAKE A DIFFERENCE:
As a Physical Therapist (PT), you will embody care and compassion in every interaction. You will collaborate with an interdisciplinary team dedicated to delivering high-quality, empathetic care that leads to positive outcomes for our patients. Your role will involve providing home-based therapy as prescribed by the attending physician, engaging with patients' families, and playing a vital role in their recovery journey. Home care offers invaluable insights into what truly aids patients in regaining their health.

HOW YOU'LL WORK:
You will deliver comprehensive skilled services as per the attending physician's orders. Educating patients and their families while ensuring patient safety will be a key responsibility. You will be accountable for accurate documentation and compliance with all legal regulations.
MAJOR AREAS OF RESPONSIBILITY:
- Plan of Care: Deliver skilled physical therapy as directed by the attending physician. Observe, document, and communicate patient reactions, changes, and progress to the physician and relevant agency staff. Conduct OASIS assessments to create a personalized care plan and adjust it as necessary.
- Patient Outcomes: Provide treatment to alleviate pain, restore function, and maintain optimal performance through various physical means, including exercise, massage, heat, and water.
- Minimize Patient Risk: Reduce re-hospitalizations by prioritizing visits for high-risk patients and educating them on preventive measures and sound decision-making.
- Family Educator/Advocate: Guide patients and their families on managing diseases or injuries throughout the physical therapy process. Instruct on the proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other assistive devices.
- Supervision: Oversee therapy services provided by PTAs and, when appropriate, nursing assistants.
- Communication/Collaboration: Maintain communication with PTAs and nursing aides regarding supervisory visit outcomes.
- Policies: Complete all clinical documentation in accordance with agency protocols and Medicare/Federal guidelines.
- Rules and Regulations: Understand and adhere to agency policies, procedures, rules, and regulations, and communicate any changes in schedule or availability to schedulers or supervisors.
- Operations: Participate in in-service training and mandatory agency meetings.
- Empathetic communicator with a positive demeanor
- Patience is essential when interacting with patients, families, physicians, and colleagues
- Meticulous attention to detail, observation skills, and adherence to instructions are crucial
- Licensed Physical Therapist in the state of employment.
- Valid driver's license and auto insurance in your name.
- Ability to meet all physical demands of the role.
