YOUR IMPACT:
As a Physical Therapist (PT), you will play a crucial role in delivering compassionate care. You will collaborate with an interdisciplinary team focused on achieving positive outcomes for our patients. Your responsibilities will include providing home-based therapy as directed by the attending physician, engaging with patients' families, and being an integral part of their recovery journey. Home care allows you to understand what truly aids in restoring health.

YOUR ROLE:
You will deliver comprehensive skilled services as prescribed by the attending physician. Educating patients and their families while ensuring patient safety is paramount. You will be responsible for accurate documentation and compliance with all legal and regulatory standards.
KEY RESPONSIBILITIES:
- Care Planning: Deliver skilled physical therapy as ordered by the attending physician. Monitor, document, and communicate patient reactions, changes, and progress to the attending physician and relevant agency staff. Conduct OASIS assessments to create a personalized care plan and adjust as necessary.
- Patient Care: Treat patients to alleviate pain, restore function, and maintain optimal performance using various physical methods, including exercise, massage, heat, and water.
- Risk Management: Reduce re-hospitalizations by prioritizing visits for high-risk patients and educating them on preventive measures and sound decision-making.
- Family Support: Counsel patients and families on their conditions and how to navigate all phases of physical therapy. Instruct on the proper use of mobility aids and other assistive devices.
- Supervision: Oversee and guide therapy services provided by PTAs and, when appropriate, nursing assistants.
- Collaboration: Communicate effectively with PTAs and nursing aides regarding supervisory visit outcomes.
- Documentation: Complete all clinical documentation in accordance with agency protocols and Medicare/Federal guidelines.
- Compliance: Understand and adhere to agency policies, procedures, and regulations, and communicate any changes in schedule or availability to supervisors.
- Training: Participate in in-service training and mandatory agency meetings.
- Empathetic communicator with a positive demeanor
- Patience when interacting with patients, families, physicians, and colleagues
- Strong attention to detail and ability to follow instructions
- Licensed Physical Therapist in the state of employment.
- Valid driver's license and auto insurance.
- Ability to meet all physical demands of the role.
