Key Responsibilities
- Oversee the referral process for patients seeking dialysis treatment, ensuring effective communication with external healthcare providers and internal departments.
- Facilitate the admission of patients, including those new to dialysis, transfers from other facilities, and patients returning after a hiatus.
- Collaborate with nephrologists, hospital staff, and social workers to gather necessary medical information and coordinate patient placements.
- Maintain meticulous records of referrals and admissions, ensuring compliance with medical protocols and attention to detail.
- Adapt admission procedures to align with organizational policies and changes in healthcare regulations.
- Participate in interdepartmental meetings to enhance process efficiency and transparency.

Qualifications
- A minimum of a 2-year college degree is preferred.
- Proficiency in Microsoft Office, particularly in developing Excel spreadsheets.
- At least one year of experience in a clinical dialysis setting, with a strong understanding of medical terminology and patient care coordination.
- Familiarity with various insurance payor types relevant to kidney disease patients is advantageous.
