Job Summary

We are seeking a highly skilled RN Care Manager to join our team. As a RN Care Manager, you will play a critical role in supporting patient care by coordinating services to optimize care delivery. This hybrid role involves working closely with the care team to craft personalized care plans, conducting regular assessments, and initiating proactive outreach to patients and clinics.

Key Responsibilities
  • Develop and implement comprehensive care plans addressing the clinical and non-clinical needs of patients
  • Conduct regular assessments to detect undisclosed medical or social issues and changes in the home environment
  • Initiate proactive outreach to patients and/or clinics to evaluate their status, offer assistance, and address inquiries
  • Manage a diverse patient panel, evaluating individual needs while adhering to NCQA standards for complex case management
  • Communicate updates to the care team and family members, identifying gaps in patient understanding of their health status and delivering tailored education and resources
  • Act as a liaison with dialysis facilities, conveying care plan updates to facilitate smooth transitions in care
  • Generate and evaluate reports using Strive data platforms to identify trends, anomalies, and areas requiring focus
Requirements
  • Associate's degree from an accredited school of nursing
  • Current Registered Nurse (RN) license in the state of practice
  • 3+ years' experience in clinical nursing
  • Current Basic Life Support (BLS) certification
  • Efficient and reliable transportation, including an active driver's license, allowing for the ability to travel across an assigned region to meet patient needs
  • Proficiency in computer skills, including Microsoft 365 Suite (word, outlook, excel)
Preferred Qualifications
  • Minimum of 3 years' experience in nephrology, ER, ICU, Cardiac, or Ambulatory Complex case management nursing
  • Certified Case Manager (CCM)
  • Experience developing strong patient and family relationships that foster engagement and best outcomes for all aspects of Strive Health's Model of Care
