The Community You Will Serve: AdventHealth Orlando

The Internal Medicine division offers clinical services such as pharmacokinetics, TPN, anticoagulation, cardiac consultations, dose optimization, transitions of care, and more, catering to approximately 300 patients daily. This position will continue to provide these essential services alongside the evening shift counterpart.

Your Role:

The Clinical Pharmacist plays a crucial role in supporting the mission of the department, hospital, and health system by preventing, identifying, and resolving medication-related issues to ensure patient safety, enhance patient satisfaction, optimize outcomes, and reduce costs. Responsibilities may include working in both centralized and decentralized settings. The role encompasses all functions of a registered pharmacist, including precise and timely dispensing, clinical interactions with patients and the healthcare team, and supervision of pharmacy technicians.

Your Contributions to the Team:

a) Pharmacy Operations – Actively engages in all pharmacist duties, participates in established work standards, and maintains proficiency in covering central pharmacy and/or satellite areas as needed. Ensures effective use of medication automation, technology, and computer systems. Manages medication inventories and oversees the preparation and dispensing of patient-specific medication doses. Supervises technician and intern activities, prioritizes workload, organizes workflow, and verifies the accuracy of pharmacy technician tasks.

b) Pharmacy Practice – Conducts thorough medication order reviews, drug profile assessments, and patient monitoring to identify, prevent, or mitigate medication-related issues. Accurately verifies and enters medication orders while proactively communicating with medical staff to resolve any discrepancies. Provides patient-centered care services, ensuring safe and timely medication therapy, and promotes cost-effective drug therapy, including participation in stewardship activities and restricted medication programs. Initiates appropriate drug dosing and manages medication-related consultations. Facilitates medication safety practices by identifying, mitigating, and reporting potential and actual adverse drug events, reactions, and medication errors. Participates in managing medical emergencies and maintains ongoing professional development.

c) Clinical Pharmacy Services – Actively engages in direct patient care programs, delivering accurate, safe, timely, and appropriate medication therapy based on patient needs. Collaborates with healthcare providers and pharmacy staff, prioritizing patient-centered care delivery.

d) Teamwork and Inter-professional Collaboration
i) Arrives on time and prepared to work to ensure continuity of care between shifts and among pharmacy staff assignments. Collaborates effectively with all healthcare team members to achieve optimal patient outcomes and demonstrates sensitivity to the perspectives of patients, caregivers, and colleagues in all communications.

ii) Participates in departmental meetings and educational activities as assigned. Supports practice advancement initiatives, clinical research projects, and stewardship efforts led by colleagues. Maintains a professional image by engaging in shared governance committees, if applicable.

e) Teaching, Training, Precepting, and Mentoring – Mentors new staff and assists in training all team members as requested. Supports experiential training for pharmacy trainees and mentors pharmacy students as needed. Demonstrates a commitment to teaching and maintains the ability to deliver effective evidence-based education to nurses, patients, students, and pharmacy team members.

f) Quality Improvement, Regulatory Compliance, and Accreditation – Contributes to quality improvement initiatives aimed at enhancing medication-related processes, pharmacy systems, or patient care programs. Supports regulatory compliance initiatives and institutional accreditation efforts.
