Job Summary:
We are seeking a highly skilled Clinical Staff Pharmacist to join our team at Health First. As a key member of our healthcare team, you will play a critical role in ensuring the safe and effective use of medications for our patients.
Key Responsibilities:
- Medication Order Interpretation: Accurately interpret and transcribe medication orders, maintaining accurate and complete patient medication profiles.
- Pharmaceutical Dispensing: Compound and dispense pharmaceuticals, including sterile, chemotherapy, and parenteral nutrition products, with precision and attention to detail.
- Controlled Substance Management: Issue controlled substances to patient care areas and maintain records as required by law.
- Drug Therapy Monitoring: Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
- Antimicrobial Stewardship: Review and interpret culture and susceptibility data for antibiotic appropriateness and recommend changes as needed.
- Clinical Documentation: Accurately and completely document all clinical activities and interventions.
- Quality Improvement: Participate in quality improvement and medication use review activities, collecting data, conducting quality monitors and inspections, and maintaining logs and records as assigned.
Requirements:
- Current pharmacy licensure
- Ability to maintain patient and department/hospital confidentiality
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
