Job Summary

San Ysidro Health Center is seeking a highly skilled Licensed Clinical Social Worker (LCSW) to join our team in a primary care setting. As a Behavioral Health Clinician, you will work closely with primary care providers to develop and implement sound treatment plans for adults, older adults, children, and their families. Key Responsibilities
  • Evaluate and provide brief, evidenced-based psychological/behavioral health interventions to patients with physical health, mental health, and/or substance abuse diagnoses.
  • Provide consultant services to clinical staff/primary care providers, including timely feedback, treatment recommendations, and progress updates.
  • Perform brief assessments of patient needs and develop negotiated care plans with patients and primary care clinicians.
  • Maintain accurate and timely documentation in electronic medical records and produce reports as required.
  • Evaluate and revise care plans as needed, with appropriate documentation and communication to relevant team members.
  • Perform crisis interventions, including suicide assessments and recommendations to medical providers and patients.
  • Work in coordination with the Chief Behavioral Health Officer/Director of Behavioral Health to provide information to healthcare providers about mental illness in the medical setting.
  • Assist in the implementation and administration of ESBIRT (Enhanced Screening, Brief Intervention, and Referral to Treatment) program model for substance abuse prevention and/or treatment.
  • Participate in meetings, including weekly multi-disciplinary/case conference/pod care team meetings, and contribute to the development of a train-the-trainer SBIRT model for primary care team members.
Requirements
  • Sensitivity to multi-cultural concerns and the psychotherapeutic process.
  • 2 years of experience in a community health/mental health setting, with experience in Behavioral Medicine/Health Psychology preferred.
  • Master's degree in Social Work from an accredited school or its equivalent.
  • LCSW issued by the state of California and a valid driver's license and access to an insured vehicle required.
  • Bilingual English/Spanish required, with excellent oral and written communication skills.
