Social Work Case Manager

Kaiser Permanente is seeking a skilled Social Work Case Manager to join our team in Silver Spring, Maryland.
Job SummaryThis is a challenging and rewarding role that requires a strong understanding of healthcare systems and a passion for delivering high-quality patient care. As a Social Work Case Manager, you will work closely with our healthcare team to coordinate patient care, identify and address social determinants of health, and provide supportive counseling and education to patients and their families.
Key Responsibilities- Collaborate with healthcare teams to develop and implement comprehensive care plans that address the physical, emotional, and social needs of patients.
- Conduct thorough assessments of patients' social and emotional needs, identifying areas for intervention and developing strategies to address them.
- Provide supportive counseling and education to patients and their families, addressing topics such as end-of-life care, advanced directives, and healthcare navigation.
- Facilitate access to community resources and services, including financial assistance, housing support, and mental health services.
- Develop and maintain relationships with community partners and healthcare providers to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance patient outcomes and experience.
- Stay up-to-date with relevant laws, regulations, and policies affecting social work practice in the Mid-Atlantic region.
- Master's degree in social work (MSW) required.
- Minimum one (1) year of clinical social work experience in a healthcare setting required.
- Licensed Independent Social Worker (District of Columbia) within 6 months of hire OR Licensed Graduate Social Worker (District of Columbia) within 6 months of hire.
- Licensed Master Social Worker (Maryland) within 6 months of hire.
- Licensed Master's Social Worker (Virginia) within 6 months of hire.
- Experience with computer software programs in a Windows environment preferred.
- Knowledge of community systems and resources in the defined service area preferred.
- Knowledge of regulatory issues for the Mid-Atlantic area preferred.
