POSITION SUMMARY:

The Social Work Case Manager plays a crucial role in collaborating with the healthcare team to ensure seamless care for patients. This position focuses on minimizing delays and denials in patient care while addressing various social issues that may affect health outcomes.

KEY RESPONSIBILITIES:

  • Act as a liaison between patients and community resources to tackle issues such as abuse, neglect, substance use, and mental health challenges.
  • Facilitate connections to financial assistance and other necessary resources for patients and their families.
  • Work closely with healthcare professionals to develop comprehensive care plans that meet the needs of patients.

QUALIFICATIONS:

  • Master's degree in Social Work is mandatory.
  • A minimum of 2-4 years of relevant experience in Social Work is required.
  • Experience in Medical Social Work is highly desirable.
  • Certification as a Licensed Clinical Social Worker (LCSW-C) is essential.
